View topic - How can I add an input form to zen cart

I would like to add an information form to one of my sites that people can input details into.

How would I do this? If anyone can tell me or give me a link, it would be much appreciated. I've been searching for a while but can't find anything. I do find sites with forms but haven't a clue how I would add them. Is there anything for zen cart.

If it's a case of copying one form and amending it, how would I do that. As you can see I haven't a clue.

I know it is a little late, but you could use PHP FormMail Generator to create your form and insert the code into an EZ page for a much easier solution.
